According to Art. 13 Regulation of the European Parliament and of the Council (EU) 2016/679 of April 27, 2016. on the protection of individuals with regard to the processing of personal data and on the free movement of such data and the repeal of Directive 95/46/EC (General Data Protection Regulation – RODO), we inform you that:

  1. The administrator of your personal data is Jakub Kozłowski, residing at: 144 Kowalska Street 51-424 Wrocław
  2. Personal data is collected and processed for the purpose of fulfilling the administrator’s legal obligations, pursuant to Art. 6 paragraph. 1(c) RODO, and to the extent that the provision of personal data is voluntary – on the basis of the consent you have given (Article 6(1)(a) RODO)
  3. Personal data shall be stored and disposed of within the time periods indicated in the uniform material list of files and the instruction on the organization and scope of operation of company archives or other laws regulating the time of data processing to which the controller is subject, and if these laws do not indicate a time period for a specific case – the data shall be stored for the shortest possible time necessary for the performance of an obligation or service
  4. Once processing is complete, personal data will not be processed for any purpose other than the original purpose of processing
  5. When obtaining personal data, the controller shall provide the data subject with information on whether the provision of personal data is a statutory or contractual requirement or a condition for entering into a contract, and whether the data subject is obliged to provide such data and what are the possible consequences of failing to do so
  6. The recipients of your personal data are:
  • persons authorized by the controller to process data in the performance of their official duties
  • Entities to which the controller subcontracts the performance of activities that require data processing (processors)

7.            The controller does not transfer or intend to transfer personal data to a third country or international organization

8.            The administrator does not use automated decision-making mechanisms, including profiling

9.            A person whose data is processed within the limits of the RODO has the right to request from the controller access to his or her personal data, rectification, erasure or restriction of processing, or to object to the processing of data, as well as to data portability and to lodge a complaint with a data protection supervisory authority, and, if he or she has voluntarily consented to the processing of personal data, the right to withdraw that consent at any time, which does not affect the lawfulness of the processing carried out on the basis of consent before its withdrawal.
